To access your medical record, please contact our Health Information Management Department. Prior to releasing copies of medical records, patients must complete an Authorization for Release of Information form.

The Authorization for Release of Information form is in PDF format and requires Adobe Acrobat Reader. If you don't have this software, go to Adobe for a free download.
